Adult Caregiver Coordinator

Reports To: Director of Programs
Location: Essex County, NJ
Job Type: Full-Time

Coordinator Responsibilities:

· Coordinate daily operations of program to ensure compliance with funding/level of service mandates, included but not limited to: maintaining client roster, completing monthly Social Assessment Management System (SAMS) reports, completing monthly and quarterly programmatic reports, monitoring units of services at a minimum of bi-weekly basis, notifying manager when units of services falls below contracted units of service.

· Provide clinical services to caregivers of older adults and older adult caregivers of children.

· Field referral calls, initial contact with clients, updating client records pertaining to the requirements and expectations provided by the Essex County Division of Senior Services.

· Maintain client roster, through electronic health recording system and other duties as requested related to client records.

· Maintain administrative contact with Essex County Division of Senior Services

· Attend all meetings, in service trainings, case conference meetings coordinated by the Essex County Division of Senior Services or Family Connections as required.

· Communicate with and market to the referral sources, with support of the Manager, to increase or maintain LOS as indicated.

· Provide information about other programs and services for which the participant may be eligible and referring the participant to the proper services as necessary.

· Engage in telephone counseling as necessary.

· Facilitation of Health Education groups.

Clinical Responsibility:

  • Provide clinical services as described in Clinician Job Description.

Requirements:

Education: Graduate of an accredited college or university with a Bachelors degree, or higher in a health related field or behavioral science field. Masters Level licensed or certified social worker or related field (LSW, LCSW, LAC, LPC, LAMFT, LMFT)

Experience: One year or equivalent of compensated work or internship with the elderly or physically disabled in an institutional or community setting. Minimum 2 years supervisory experience.

Knowledge: Knowledge of older adult populations needs and treatment. Knowledge of community resources, assessments, care plans, progress notes, and computer database systems for older adults. Knowledge of appropriate interventions in a crisis and non-crisis situation.

Skills and Abilities: Ability to assess client’s needs; assists in obtaining concrete services and the ability to complete all paperwork in a timely manner. Good written and oral communication skills. Has the ability to create a warm, caring, non-threatening therapeutic atmosphere. Has the willingness to work with individuals from diverse backgrounds. Understands professional ethics and issues of confidentiality. Has the ability to work on a multi-disciplinary team. Has the ability to use a computer database system.

Work Environment/Physical Demands: The individual must possess the ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, and lifting of up to or in excess of 50 pounds.
